Child Enrollment & Medical Information

This form provides basic enrollment information on the enrolling family as well as Emergency Transportation information

Child’s Medical Statement

This form is required by ODJFS and must be completed by your child's pediatrician 

​

Basic Infant Care Form (6wks. – 18mo.)

This form provides the basic care information for your infant (younger than 18 months) that you want our infant teachers to know

​

Health Care Plan

This form is to be used for any chronic illness in which needs to be addressed, such as allergies or Asthma

​

Administration of Medication

This form is to be used if your child requires any medication, including diaper cream and Sunscreen

​

Media Waiver

This form lets us know if you give us permission or not to use images of your child in various media, such as here on our website

 

Parent Handbook

This is our Parent Handbook, and includes an acknowledgement on the very last page which must be signed and returned back to us. The rest of the handbook is yours to keep
